First Visit Instructions

Your time is valuable to us. We understand that you may have a busy schedule, and we want to be able to complete your visit in a timely manner so you may get back to what is important to you.

  • Please begin the process for your treatment by completing the Online Patient Registration. This will improve the efficiency of your appointment and allow us to see you faster the day of your scheduled appointment. The information needed in this process includes the following: Insurance carrier, who the insurance is under (possibly his/her social security number and date of birth), demographic information, name of the patient, parent or guardian if the patient is a child, medical history including any current medications and allergies.
  • Please make sure that you have your referral form and any x-rays provided by your dentist to allow us to properly prepare for your visit. If this information is not available when you arrive to our office, it will cause a delay for us to begin your appointment or you may need to reschedule.
  • You will need your insurance cards, photo identification, and preferred method of payment.
The First Visit: Consult or Surgery?

Depending on the surgical procedure and your medical history, a consultation visit may be required prior to the procedure. More complex medical histories such as heart and lung conditions, cancer treatment, and age may require a consultation to provide safe care for you. Coordination with your doctors and other specialists may be needed prior to your surgery and anesthesia.

If you are visiting our office for a dental implant procedure, this will require a consultation visit as well as additional appointments to allow us to plan your surgery specifically for you.

For minor procedures and relatively healthy individuals, your first visit may be both consultation and the surgical procedure. Our staff will help to make this decision when you contact our office for an appointment.

New process for checking in to the office:

Given the recent COVID-19 pandemic, we have taken measures to ensure you, our team and our community is as safe and healthy as possible. We ask that if you are not feeling well, have symptoms of COVID-19, or have been notified that you were in contact with a COVID-19 positive or presumed positive person, call and let us know. We will reschedule your appointment in the near future.

We have been limiting the number of people in the office at a given time, modified our waiting room to provide distancing, and installed hand sanitization stations for our patients. Please wear your face covering at all times in the office. After the visit is completed, one of our staff with review post-operative instructions and the patient will be discharged.
