When you provide a mobile telephone number during patient registration or surgical check-in (including a mobile number for a patient, parent/legal guardian, or designated post-operative recovery driver/escort), we use that phone number solely to send transactional healthcare communications, including:

  • Appointment reminders and two-way schedule confirmations.
  • Surgical recovery pickup notifications (“Ride Ready”) alerting a patient’s designated driver when the patient is ready for pickup.
  • Direct customer care responses initiated by our practice staff.

Opt-Out & Support: You may opt out of receiving SMS text messages from Hartman Oral and Maxillofacial Surgery P.C. at any time by replying STOP to any message. We collect only the information necessary to provide oral and maxillofacial surgical care and coordinate appointments:

  • Health & Insurance Information: Medical/dental history, referring doctor records, radiographs, insurance subscriber details, and signed clinical/financial consents submitted through our HIPAA-encrypted portals.

4. HIPAA Notice of Privacy Practices

Protected Health Information (PHI) collected by Hartman Oral and Maxillofacial Surgery P.C. is governed by the Health Insurance Portability and Accountability Act (HIPAA).
